The problems on the Titanic were systemic. The failure if the lookout was just the tip of the iceberg.
It was 3 am (dark) and it was cold (not warm) he was probably tired (sleepy) and didn't pay attention and even if he did he probably didn't see it
@lost larry , you are kinda right cold and tired aside that nights weather caused a sea mirage raising the visible skyline hiding the iceberg until it was too late even with binoculars he wouldn't have been able to see it coming people have got to stop blaming the lookouts
The problem want him. The person steering the ship panicked and turned the wheel the wrong way. Frederick did his job just fine.

@Neutral Sins, it's was a combination of causes, one of which was the fact that the lookouts were not given binoculars (again not his fault).
@I like cheese , actually the main look out was let go before the ship set off and the binoculars were in his locked locker. He had the key. They had no way of knowing it was out there.

@Kinky Dalek, The situation is obviously very tragic. But it's still possible to see those little ironies in things, even bad things. The key is to use tact when you do see the irony, and know the right time and place to share it.

Just posted this on my Facebook and the girl I really like liked it almost immediately
I read somewhere that he admitted that he (or someone else) forgot to bring the ship's binoculars.
Sounds like he looked out for himself
